I/We agree that the aforesaid personal data provided, including the name; contact details, including telephone number, fax number and email address; date of birth; gender; school; occupation; marital status and address. etc. can be used by Petite Princess Academy of Dance and all relevant persons in Petite Princess Academy of Dance or other persons or organizations providing services to Petite Princess Academy of Dance for any of the courses, programs, application process and promotion of Petite Princess Academy of Dance.

(b) 本人謹此聲明在本申請表填報的全部資料均屬正確無誤，並完全明白及同意填報之任何資料將會在小公主舞蹈學院的招生過程中作參考之用。

I/We declare that all information given in this application is, to the best of my knowledge, accurate and complete. I fully understand and agree that any of this information will be used in the admission process of any course or program offered by Petite Princess Academy of Dance.

I/we understand that, upon the registration in a course or program in Petite Princess Academy of Dance, the information contained in this application will become part of the student record and may be used for all purposes relating to the student’s study in Petite Princess Academy of Dance.

(d) 本人明白小公主舞蹈學院可能使用本入學申請資料作統計及分析用途，本申請表及有關的個人資料會於收生程序完結後銷毀。然而，若本人於申請表表示願意收到小公主舞蹈學院的資訊或推廣，則本申請表及有關的個人資料將被保留。

I/we understand that Petite Princess Academy of Dance may use the application data for statistical purposes. The application form and other related personal information will be disposed of after the completion of the admissions exercise. Nevertheless, if I/we have indicated in the application form that I/we wish to receive any information or promotion about Petite Princess Academy of Dance, the application data will be retained.

個人資料（私隱）條例──私隱政策聲明

小公主舞蹈學院 (「我們」) 在收集、保存、使用及傳送個人資料時，尊重法律賦予閣下的私隱權利；而本「私隱政策聲明」 (「本聲明」) 旨在說明我們處理個人私隱的方法。我們的政策是要遵守中華人民共和國香港特別行政區(「香港」)法例第486章《個人資料(私隱) 條例》 (「條例」)。以下將詳細說明我們處理閣下之個人資料的政策及方法，請仔細參閱及理解。本聲明適用於我們的網站(「本網站」)。我們有權按需要隨時修訂或另行更改本聲明。在本聲明中，「個人資料」一詞採用條例對該詞的定義。

We shall fully comply with the obligations and requirements of the Ordinance. Our officers, management, and members of staff shall, at all times, respect the confidentiality of and endeavor to keep safe any and all personal data collected and/or stored and/or transmitted and/or used by us, or on our behalf.

We shall endeavor to ensure all collection and/or storage and/or transmission and/or usage of personal data by us shall be done in accordance with the obligations and requirements of the Ordinance.

Where an individual legitimately requests access to and/or correction of personal data relating to the individual, held by us, then we shall provide and/or correct that data in accordance with the times and manner stipulated within the Ordinance.

所收集的個人資料類別

我們持有的個人資料可分為三大類別：

1. 學員/ 兒童的個人資料包括（但不限於）：姓名、性別、出生年月及聯絡詳情，包括電話號碼、傳真號碼及電郵地址；國籍；就讀學校。

3. 僱員的個人資料包括（但不限於）：姓名；性別；聯絡詳情，包括電話號碼、傳真號碼、住址及電郵地址；年齡；出生日期；身份證號碼；銀行戶口號碼；婚姻狀況。

Types of Personal Data collected by Petite Princess Academy of Dance may include, without limitation:

Personal Data of Members / Children, such as name, gender, month and year of birth, contact details including telephone number, fax number and email address, nationality and
Personal Data of Parents / Guardians, such as name, gender, contact details including

telephone number, fax number and email address, occupation and marital status.

Personal Data of Employees, such as name, gender, contact details including telephone number, fax number, address and email address, age, date of birth, I.D card no., bank account number and marital status.

收集個人資料的目的

我們所收集的資料的目的如下（包括但不限於）：

1. 學員/兒童的資料：所收集的資料能作以下用途，確認你所報讀參加我們的舞蹈課程、與你聯絡有關參與我們之表演、活動、處理你的投訴、查詢或意見、審批獎學金、慈善推廣、學術研究、統計、市場分析、一切行政用途或提供更適合你及/或按個人需要而設的服務、協助及資訊。

2. 家長 / 監護人的資料：所收集資料能作以下用途，確認你為子女/兒童所報讀參加我們的舞蹈劇課程、與你聯絡有關參與我們之表演、活動、處理你的投訴、查詢或意見、審批獎學金、慈善推廣、學術研究、統計、市場分析、一切行政用途或提供更適合你及/或按個人需要而設的服務、協助及資訊。

3. 僱員的資料：進行招聘、人力資源管理、聯絡、安排員工進修及活動、勞工保險及醫療保險。

在我們的報名表格，以及網上報名表，所收集資料項目分為必須提供的個人資料及自願提供的個人資料，所有註明（*）的欄目，屬必須提供的個人資料，如未能提供有關資料，會令我們不能提供相應的服務或協助。

The purposes of Personal Data collection may include, without limitation:

Personal Data of Members / Children: The data may be used to confirm your application and enrollment to our musical theatre courses, to contact you for our performances and activities, to process your complaints, enquires of feedback and scholarship approval, to promote charity activities, to use for academic research, statistical or administrative purposes, and/or better tailor the type of information, service and assistance presented to
Personal Date of Parents / Guardians: The data may be used to confirm your children’s application and enrollment to our dancing courses, to contact you for our performances and activities, to process your complaints, enquires of feedback and scholarship approval, to promote charity activities, to use for academic research, statistical or administrative purposes, and/or better tailor the type of information, service and assistance presented to you.

Personal Data of Employees: The data may be used in recruitment, human resources manage-ment, staff contact, training and activities arrangement, and processing of employees’ compensation and medical insurance.

查閱及改正個人資料的權利

根據及按照「條例」的條款，任何個人有以下權利：

Subject to the paragraphs below, all personal data held by us will be kept confidential but we may, where such disclosure is necessary to satisfy the purpose, or a directly related purpose, for which the data was collected provide such information to the following parties:

Companies / groups / organizations under common control with us, such as our branch of theatre or arts group;
Any of our associated companies / groups / organizations, as cooperation in any kind of business or project, such as jointly present or co-present of performance, or our members or employees participating in the performance and activities.

Any person or companies / groups / organization, who is acting for or on our behalf, or jointly with us, in respect of the purpose or a directly related purpose for which the data was provided, such as PR firm employed by us; and

Any other person or companies / groups / organization, which is under a duty of confidential-it to us and has undertaken to keep such information confidential, provided such person or company has a legitimate right to such information.

Personal data may also be disclosed to any person or persons that have a right under the Ordinance and any applicable law to gain access to such information provided they are able to prove their authority to access such information. For example, if we were served with a court order demanding certain customer information then we would disclose the information to the duly appointed officer of the court or such other persons as the court orders.
